If you have any special requests or comments, please discuss them with someone at the office. Be sure to mention hometowns of persons named in article.

Please, keep in mind that these items run as space is available. Therefore, the earlier you return this form the sooner it may be published. However, it still may not be published for several weeks.

Photos are accepted and can be black and white or color prints. Please submit photos in formats of *.jpeg or *.jpg. Save these photos at a minimum resolution of 200 dpi.
